The Skilled Nursing Facility Hospitalist acts under the direction and supervision of a Center, Associate, or Corporate Medical Director. The SNF Hospitalist provides direct patient care in accordance with Company practice policies, relevant state and federal regulations, and within the limits of their experience, training and privileges. This position provides services in the SNF setting.

Represents the Company in a professional manner, following all Company policies and procedures
Uses, protects, and discloses patients’ protected health information (PHI) only in accordance with Health Insurance Portability and Accountability Act (HIPAA) standards
Recommends, orders, and evaluates diagnostic studies and therapeutic interventions
Conducts comprehensive or episodic health assessments of patients with acute or chronic health problems in order to establish medical diagnoses
Examine, diagnose, and treat, without delay, (to include direct physical contact with):
Patients who may have non-specific symptoms suggestive of flu or other respiratory illnesses, in any location (including for example an office setting or hospital setting such as reverse isolation in a hospital or SNF, ICU, etc.)
Febrile patients with not yet diagnosed illnesses or where the offending organism has not yet been identified
Patients in reverse isolation due to neutropenic sepsis, host-compromised illnesses, or where the offending organism is not yet known (bacterial, viral, fungal, parasitic, etc.)
HIV-AIDS patients who may be sick with infection or who may be asymptomatic and not yet identified; with SNF or hospital patients with or suspected of Tuberculosis, MRSA, viral illnesses, or respiratory illness
Patients who may be ill (with or without fever) or who may have been diagnosed with potentially or known communicable or contagious diseases, or conditions to include, but not limited to the following: Flu or other respiratory diseases, Pneumocystis Carini pneumonia, or viral or fungal diseases to include CMV, herpes, chicken pox, shingles, TB, etc.
Draw blood, start intravenous lines, change infected wound dressings, drain skin abscesses and examine infected wounds in office, hospital or SNF setting on occasion
Monitors medical, radiographic, laboratory and any other diagnostic parameters associated with patient care
Monitors therapeutic procedures, assess and manage/modify follow-up plans
Prescribes and regulates medications in accordance with Florida Statutes
Records and documents health appraisal data necessary to maintain and coordinate health care needs and maintain a medical record
Consults and refers to other health care providers as indicated by individual patient needs
Conducts patient assessments including triage; educates patients on preventive health care (to include family/next of kin) and acute care follow-up as needed
Diagnoses and treats emergency conditions in accordance with BCLS guidelines as indicated
Physically rounds at skilled nursing facilities when assigned; takes evening/weekend call on a rotating basis, and provides timely telephonic and in-person care to patients including in the middle of the night and or weekends without delay
Treats and physically has contact with patients without exception in an office, hospital, or SNF setting, in all areas of the facility without exception, to include ER, reverse isolation, ICU, dialysis, etc.; not delay or avoid seeing any patients in these settings with specific symptoms
Performs other duties as required

MD/DO degree from accredited medical school and residency in Family Medicine or Internal Medicine
Current and Unrestricted license to practice medicine in the State of Florida
Current and unrestricted DEA certificate
Board eligible or board certified. If eligible, able to achieve and maintain certification within 2 years of joining the company
Full COVID-19 vaccination is an essential requirement of this role. Candidates located in states that mandate COVID-19 booster doses must also comply with those state requirements. Managed care experience
Ability to read, write, speak and understand the Spanish language

If the hired individual resides in Florida this position requires the AHCA Level II background check (fingerprinting) by the State of Florida for all clinicians that have direct face to face contact with members OR employees who will have access to confidential patient data and will require renewal every five years.
