The Advanced Practice Provider will deliver outpatient care for adults with nephrology related diagnoses. Position Details: Provides outpatient clinic care in collaboration with physicians and other members of the healthcare team for nephrology care.Maintains an advanced knowledge base and skills in health promotion and maintenance, disease prevention, physical and psychosocial assessment, and management of health care individuals.Performs care including but not limited to evaluation, assessment, and management of renal disease in the ambulatory care set
